PENILE CANCER, EPIDEMIOLOGICAL DATA CAN DETERMINE THE DISEASE TREND AND HOW TO CONDUCT PREVENTION AND TREATMENT?
Resumo
Objective
The aim of this study is to investigate the socio-demographic data, staging, first treatment received, histological type and post-treatment evolution of patients with penile cancer in Brazil during the last 20 years. Thus, establishing a pattern of disease involvement and designing better ways to diagnose, treat and prevent of penile cancer.
Methods:
We obtained data from the database of National Cancer Institute, collected using the International Classification of Diseases (ICD-10) C60, between 2000– 2020. Statistical analysis was performed. The groups were compared for differences between 2000 and 2020.
Results
During the period there were 14,466 cases of PC, tabulated in the 367 hospitals that make up the INCA tabulator, with an average of 723 cases/year. There was an increase in the number of cases until 2014 (925/year), followed by a decrease in the total number, in 2020 with 360 cases. 58% were brown, the age group most affected was between 60-69 with 3445 cases (23%). Regarding the level of education, 50% of the patients did not complete basic education (semi-illiterate). Alcohol consumption 26%. Smoking was more expressive, 30% of smokers and 17% of former users, with tobacco use being significant (p<0.02), with an increase in the rate of PC patients exposed to smoking over time. The higher incidences were observed in regions (northeast) with lower Human Development Index (HDI), with 45% of cases. First treatment was surgery in 70% of patients, followed by chemotherapy + surgery (8%), chemotherapy alone (7%), surgery and radiotherapy (4%), surgery + radiotherapy (4%), radiotherapy alone (5%). Most prevalent histological type was squamous cell carcinoma (SCC) (87%), Verrucous carcinoma (3.7%), adenocarcinoma (1.6%), basaloid (1%).
Conclusion
PC was more prevalent in older patients, in regions with lower HDI. It is significantly related to tobacco use and low educational level, with an increase in the incidence rate in this population. Advanced disease is increasing significantly, as are cases in young patients. Screening and prevention is necessary in this population for early diagnosis and treatment.
