COMPOUNDING AND HANDLING OF ONCOLOGY DRUGS: DEVELOPMENT OF AN ORAL HYDROXYCARBAMIDE SOLUTION FOR “OFF-LABEL” USE IN PEDIATRICS DEPARTMENT

Resumo

Introduction: Hydroxycarbamide, available as tablets, is a pharmacological agent for fetal hemoglobin induction such as sickle cell anemia. The need for alternative dosage form options for patients unable to take tablets led hospital pharmacies to prepare solutions and suspensions. There is a choice of an off-label formula that can be reproduced for use in patients, who cannot use the Hydroxycarbamide in tablets ingested orally. The information provided is intended for professionals in oncology with the correct administration of oral solids solution. Objectives: The objective of this study was active search carried in November 2019 and the key word “extemporaneous formulation, off-label use, stability of oral suspension of hydroxyurea, hydroxycarbamide oral suspension” out on integrative literature review in the PubMed, timeless research, without language limitation. Methods: Articles published in full that answered the guiding were included in research. Results: These are the two formulas for the preparation of Hydroxycarbamide oral solution. Formulation nº 01: dissolve 4 capsules of Hydroxycarbamide 500 mg in 50 mL of purified water for at least 20 minutes. Administer using an oral syringe. Rinse the nasogastric tubing before and after the medication is administered. Formulation nº 02: dissolve 8 capsules of Hydroxycarbamide 500 mg, oral solution q.s.to 100 mL. Grind up the powder of capsules, add the oral solution and mix well. Stability is 15 days under refrigeration and room temperature. Oral solution formula: methylcellulose 1% (50 mL) and syrup NF q.s.to 100 mL. Conclusions: Patients can use these formulas prepared from tablets, and liquids provide maximal dosing flexibility and it is possible to use a single formulation over a wide age range and this strategies were effective in prevent medication errors in Pediatrics and Neonatal units, contributing to promote patient safety and pharmaceutical care.
